How to apply for a birth certificate in person?
Visit one of our convenient offices and request the certificate in person. You will have to present your current state issued ID (or alternative identification - 24 KB), pay the fee by cash, with your personal check, MasterCard or Visa (credit in person only) and your order will be processed while you wait. How do I apply for a birth certificate through the mail?
To order by mail, download the birth application (86 KB), complete the form and send it in with a copy of both sides of your state issued ID. Make a check or money order payable to Vital Records. More information.

How can I expedite my Utah birth record?
Salt Lake Valley Health Department does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. How can I expedite my out of state record?

Note: VitalChek and other companies you find on the internet charge extra fees to process birth certificate requests. If you request your certificate through our office by mail, you will only pay the regular fee and it is promptly returned to you.
