Birth and Death Certificates

If you have ever had a baby, applied for a passport, enrolled a child in school, obtained a driver license, or needed a death certificate of a family member, your efforts in all probability involved acquiring a certified vital record.

Salt Lake Valley Health Department Vital Records offices are conveniently located throughout the Salt Lake Valley and are open Monday through Friday. Your request for a birth or death certificate is processed while you wait at any of our offices.

When requesting records through the mail, carefully follow the instructions outlined on our Mail Requests page. Requests made through the mail are promptly processed.

Although we have birth records from 1951 to the present for the entire state, other birth and death records are limited to within Salt Lake City limits, or areas within Salt Lake County. Please see our list of available records.
